The District of Columbia, Department of Health (DC Health) is requesting proposals from qualified applicants to provide services in the program and service areas in this Notice of Funding Availability (NOFA).
DC Health’s strategic priorities are to:
- create a culture of health and wellness,
- address social determinants of health,
- close the chasm between clinical and public health,
- strengthen public and private partnerships, and
- implement data-driven programming.
Funding Information
- Anticipated # of Awards: 4
- Anticipated Amount Available: $6.7 million
- Floor Award Amount: $650,000
- Ceiling Award Amount: $1.5 million
Outcomes
- Outcome Area 1: Breastfeeding Education and Support
- Outcome Area 2: Nutrition Assessment, Counseling, and Education
- Outcome Area 3: Systems Alignment (Community-Clinical Linkages)
- Outcome Area 4: Case Load: Enrollment, Participation, and Retention, and Redemption
Target Populations
Applicants shall provide services to income-eligible District residents who are:
- Women
- Pregnant (during pregnancy and up to six (6) weeks after delivery or the end of the
- pregnancy)
- Postpartum (up to six (6) months after delivery)
- Breastfeeding (until the infant’s first birthday)
- Infants up to 1 year of age
- Children 1-5 years of age
Eligibility Criteria
- Nonprofits having a 501(c)(3), Federally Qualified Healthcare Centers (FQHCs), or hospitals located and licensed to conduct business within the District of Columbia.
- The following are eligible organizations/entities who can apply for grant funds under this RFA:
- Nonprofit organizations
- Private organizations
- Public organizations
