The Western NSW Primary Health Network (WNSW PHN) is offering funding support for the establishment of up to 20 Aboriginal Health and Wellbeing Yarning Groups across the region to strengthen culturally safe, community-led health initiatives focused on chronic disease prevention and management.

Each group will aim to promote health literacy and awareness of chronic disease risk factors, prevention, and management; strengthen connections to culturally appropriate primary health care services; facilitate peer support and shared learning in a culturally safe environment; and encourage holistic approaches to health and wellbeing, recognising the social, emotional, spiritual, and cultural determinants of health.

Funding of up to $12,000 (excluding GST) will be available for each eligible organisation to establish and operate a yarning group. These funds aim to support activities that promote learning, connection, and practical strategies for improving community health outcomes.

Through this grant program, WNSW PHN seeks to empower Aboriginal communities to take an active role in their health and wellbeing by creating spaces where conversations around chronic disease prevention and management can thrive. The initiative underscores the importance of culturally grounded approaches, acknowledging that effective health strategies are built on trust, inclusion, and community connection.

Eligible applicants include Aboriginal Community Controlled Health Organisations (ACCHOs), Aboriginal organisations or businesses with a valid ABN, local Aboriginal committees operating under formal auspice arrangements, and community groups working in partnership with First Nations communities. Applications from non-Aboriginal entities must include letters of endorsement from Aboriginal Community Controlled Organisations or recognised Aboriginal community groups to ensure cultural integrity and collaboration.
