Call for Proposals: Drivers of Health Grants (United States)

The Physicians Foundation’s Drivers of Health Grants seeks five medical associations to help physicians understand and integrate Drivers of Health (DOH) into their patient care.

The goal is to support efforts that improve health outcomes and reduce healthcare costs by addressing non-medical factors such as food insecurity, housing instability, and transportation issues that influence a patient’s health. This grant program aims to shift the focus from food quantity to nutritional quality, enabling physicians to connect patients with resources for better diet and health.

Proposals should be implemented over a one- to two-year period, with a maximum funding limit of $75,000. Grantees will be asked to identify and compile a guide of existing resource lists from social service organizations. They will also have the chance to participate in virtual check-ins and educational sessions sponsored by the Physicians Foundation. An online community platform is available for current and past DOH grantees to share best practices.

To be eligible, an organization must be a medical association or foundation that can recruit a group of willing physicians to integrate DOH screenings and resource navigation into their practices. While the Foundation generally limits organizations to one grant at a time, this policy is waived for this RFP, though organizations with an existing DOH grant are not eligible to apply.
