The Administration on Aging (AoA) within the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS) forecasts the possible availability of Fiscal Year (FY) 2017 funds to make three-year grants to approximately 6-8 entities to bring to scale and sustain evidence-based programs that empower older adults and adults with disabilities to better manage their chronic conditions.
Goals
- Significantly increase the number of older adults and adults with disabilities in underserved areas and populations who participate in evidence-based self-management education and support programs to empower them to better manage their chronic conditions.
- Implement innovative funding arrangements, including contracts and collaborations with one or more sustainability partners, to support evidence-based self-management education and support programs during and beyond the grant period. Additionally, embed programs into an integrated, sustainable evidence-based prevention program network via centralized, coordinated processes.

- Private institutions of higher education
- Native American tribal governments (Federally recognized)

- Nonprofits having a 501(c)(3) status with the IRS, other than institutions of higher education

- Public and State controlled institutions of higher education
- State governments
- Native American tribal organizations (other than Federally recognized tribal governments)
