Centura Health Equity & Advancement Fund Grant Program – US

Applications are now open for the Centura Health Equity & Advancement Fund Grant Program to provide funding opportunities for initiatives that have a direct correlation with social justice and health equity across Colorado and western Kansas.

In formalizing their fiscal year priorities in 2020-21, they released the Social Justice Framework to guide intentional and strategic actions to advance efforts in changing both internal and external structures and systems to drive meaningful change in serving the community and delivering whole person care.

The Centura Health Equity & Advancement Fund was launched in 2021 in support of Centura Health’s commitment to Diversity and Inclusion across their vibrant communities in Colorado and Western Kansas. Their commitment to social justice lies in their Core Values, their Mission to extend the healing ministry of Christ, and from their Adventist and Catholic founders, who teach them that all people are made in the image of God and so possess an equal and inalienable worth.

The Social Justice Framework is focused on five areas: Recognition and Support, Education and Development, Human Resources, Health Disparities and Community Impact. The Centura Health Equity & Advancement Fund is part of the Community Impact initiative.

Funding Information

Projects can range from $5,000 to $250,000 and must serve specific diverse communities and/or populations, focus on diverse caregivers, and leverage funding from other sources to increase social impact.

Funding Criteria

• Funding criteria for the Centura Health Equity & Advancement Fund include projects that address social justice issues by advancing initiatives that extend health equity across Colorado and Western Kansas.
  • Applicants should ensure that their proposed projects:
    • Address a part of the Social Justice Framework and/or one or more prioritized significant needs in the local hospital’s Community Health Needs Assessment and Implementation Plan.
    • Serve identified vulnerable or underserved populations to help address health disparities
    • Collaborate with other organizations; joint applications are encouraged but not required
    • Integrate one or more of the following principles:
      • Focus on disproportionate unmet health-related needs
      • Emphasize primary prevention and address underlying causes of health problems
      • Contribute to a seamless continuum of care
      • Build community capacity
      • Demonstrate collaboration
      • Address social determinants of health to improve health outcomes
    • Identify – and plan to evaluate and report on – specific and measurable outcomes
  • Proposals addressing needs exacerbated or created by the coronavirus pandemic (including physical or mental health, and health-related social, economic and human needs) are encouraged.

Requirements

• Projects must have a direct correlation to advancing social justice and health equity, using their Social Justice Framework as a guide or resource
• Projects must serve specific diverse communities and/or populations, focus on diverse caregivers, and leverage funding from other sources to increase social impact
• A brief written report and financial report will be required upon completion
• Application submissions can be from $5,000 – $250,000
• One application per collaborating organization
• Preference will be given to projects that have direct correlation to social justice and equity, directly serve diverse populations and leverage funding from other sources.
• Ineligible activities/organizations include:
  • Capital projects
  • Debt retirement
  • Scholarships/endowments
  • Studies/research
  • Political campaigns
  • Debt retirement or operating deficit
  • Public agencies including health departments and local governments
  • Physician-owned (or physician immediate family-owned) organizations where physician may be in a position to refer

Eligibility Criteria

• 501 (c)(3) tax exempt nonprofit organizations
• Community organizations using a nonprofit 501(c)(3) as a fiscal sponsor. If a non-profit organization cannot accept Federal funds directly, a letter must be included in the application stating that in the event the application results in an award, the award funds must be administered through a fiscal agent organization on their behalf. This agent must be identified in the letter from the applicant and the letter must be countersigned by an authorized representative of the fiscal agent organization. The letter must include the fiscal agent’s point of contact, address, telephone number, fax number and e-mail address.

• Must be operating in Colorado or Kansas and within 50 miles of a Centura facility
