The Department of Health and Human Services (DHHS) and Office of the Assistant Secretary for Health is seeking applications for the Accessing Social Determinants of Health Data through Local Data Intermediaries Initiative.

It is increasingly recognized that local environmental and various socioeconomic factors, also known as SDOH or social needs, are critical contributors to health and well-being at the neighborhood level.

SDOH are the conditions in the environments where people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning, and quality-oflife outcomes and risks.
