Missouri Foundation for Health (MFH) is currently inviting organizations to submit concept papers for ideas that offer a unique opportunity to address health for Missourians in ways that create lasting impact.
They are interested in those ideas that explore or implement ways to improve, challenge, and disrupt the systems or circumstances.
The Opportunity Fund enables to respond to emerging issues and test new or innovative approaches that may be unique to the state. Ideas can also be based on existing, promising, or effective models that can be tested with new audiences or geographies. Projects may also test the viability and sustainability of an idea on a small scale as a proof of concept.
Eligibility Criteria
Applicants must be nonprofit or governmental entities and meet the general MFH funding guidelines.
Concept Paper Requirements
- Application Acknowledgement. Complete and submit the one-page acknowledgement form.
- Concept Paper Narrative. In no more than two 1.5-spaced pages, please concisely consider the following. (Note: If invited to submit a full proposal, additional details about the approach, major activities, timeline, organizational history, and partner roles will be requested. For this concept, it is important to focus on describing the idea and emphasizing how it will specifically examine, challenge, improve, or disrupt the systems that allow inequities to exist.)
- What? Provide a brief synopsis of the proposed idea and the target audience.
- Why? Why is this needed and what is the change that will be created? Why is now the right time to do this? How does this project and timing fit into the current landscape? Describe the organization’s capacity to implement a project of this scale and scope.
- To what end? What would success look like? What are the implications of this work beyond this specific idea? How does this idea challenge “business as usual” in this field and promote equity? Consider the systems-level potential for this idea. How does your project disrupt the system that holds this issue in place?
- With whom? If anticipated, please share any identified partners. A brief list of partner names is sufficient.
- How much? Please complete and submit the concept paper budget listing the estimated expenses for the project with a brief explanation (A more detailed budget will be required if a full proposal is requested).
- Annual audit report or IRS Form 990. Most recent audit report or IRS Form 990. Applicants using a fiscal agent must submit financial documents for both the applicant organization and fiscal agent.
- Conflict of Interest Disclosure. Complete and submit the disclosure form.
Additional Considerations
The Foundation reserves the right to:
- Request additional information from any or all applicants
- Conduct discussions with applicants to ensure full understanding of, and responsiveness to, the application requirements
- Request modifications to a respondent’s application prior to final award to ensure alignment of project elements with the core values, mission, and operating standards of the Foundation
- Approve subcontractors proposed or used in carrying out the work
- Reject any or all applications submitted
