The Robert Wood Johnson Foundation is seeking proposals from organizations who can serve in the role of a coordinating office to enhance hospitals’ role in population health. The Foundation (RWJF) is committed to building a Culture of Health in America, which includes fostering cross-sector collaboration to improve population health, well-being, and equity.
RWJF will work in partnership with an external coordinating office to expand the impact and sharpen the strategy of the Foundation’s efforts to enhance hospitals’ role and investment in population health and the social determinants of health. The coordinating office will work with program officers at the Foundation and will maintain relationships with RWJF grantee organizations and other relevant organizations and leaders. The coordinating office is expected to embrace a collaborative learning approach that will help link grantees to one another and amplify what grantees and other organizations are accomplishing and learning about hospitals and health systems across the United States that are focused on total population health.
In addition, it is important that the coordinating office can apply innovative approaches to monitor progress, national and local trends related to advances in population health and shifting political environments; identify strategic opportunities and gaps (among networks, grantees and others) for the Foundation and the field; and as necessary convene others to generate insights and action; align strategies, and support one another. Through activities and relationships, the coordinating office should improve the Foundation’s ability to learn from the field, synthesize and disseminate lessons from across strategically aligned investments, and make contributions that will help broaden the Foundation’s perspective of the field and inform its decisions about possible program development in two of the Foundation’s thematic focus areas, Healthy Communities and Transforming Health and Health Care Systems.
Award Information
- Up to $800,000 in funding will support one or a pair of collaborating organizations.
- If applying as a pair, one organization should act as the fiscal sponsor and primary grant recipient, and the secondary organization should subcontract with the fiscal sponsor.
- Project funding will support the coordinating office for a period lasting up to 24 months. The Foundation may consider continued support for the activities of the coordinating office after this period, contingent upon internal strategic priorities and appropriate progress toward agreed-upon deliverables within the first 18 months.
- Program start date will be June 15, 2017 and continue through June 14, 2019.

- March 27, 2017: Finalists notified of funding recommendations.

Eligibility Criteria
The Foundation seeks proposals representing a range of organizations and/or institutions with reach across sectors and national geography.
Eligible applicants:
- Must have existing organizational infrastructure that indicates sufficient capacity for this work, including at least financial infrastructure and staff with experience and expertise to perform the functions outlined in the section titled “Purpose” above. (A grantee may hire or contract services).
- May submit a proposal to work independently or with one partner organization, however each collaborating organization must be described in detail in the proposal and only one organization may represent the collaboration and be the lead contact in the application process;
- Must be based in the United States or its territories.
Preference will be given to applicants that are either public entities or nonprofit organizations that are tax-exempt under Section 501(c)(3) of the Internal Revenue Code and are not private foundations or Type III supporting organizations. The Foundation may require additional documentation.
