The Administration for Community Living is pleased to announce applications for No Wrong Door Community Infrastructure Grants Program.
The purpose of this initiative is to support the development and enhancement of Network Lead Entities (NLEs) which are providing key access functions within a community such as coordination of information and referral, screening, care coordination, care transitions, eligibility and enrollment, and person centered planning.
A growing share of community-based organizations (CBOs) are coordinating or participating in community-based networks of various types and orientations in order to create and respond to an increasing demand from health sector organizations that seek services and supports at volumes, scopes, and geographic areas larger than a single CBO can typically meet.
This has given rise to the emergence of the community integrated health network (CIHN) and the network lead entity (NLE) that resides at the CIHN’s core.
The intent of this funding opportunity is to provide funding directly to NLEs to build their network maturity and align their service capabilities in order to contract with health care sector entities (e.g., accountable care organizations, health plans, managed care organizations, hospitals, health systems, and more) to coordinate and streamline access to all health and social service options so that individuals can obtain the care they need through a single intake, assessment, and eligibility determination process.
The processes of the leading NLE can be organized into five categories or domains:
- Leadership: Strategic Planning, Organizing the Network, Controlling Network Performance, Leading Across Organizational Boundaries;
- Finance: Capitalization, Cost Management, Pricing, Making the Business Case;
- Business Development: Assessing the Market, Positioning the Network’s Services, Communicating Value Externally, Building a Health Sector Partner Base;
- Network Development and Support: Communicating Value Internally; Vetting, On boarding, and Assistance; Monitoring and Managing Performance, Retaining Network Service Providers; and
- Network Administration: Strengthening Information Systems, Managing Contracts, Meeting Compliance Standards, Billing and Payments.

- Native American tribal organizations (other than Federally recognized tribal governments)

- Nonprofits having a 501(c)(3) status with the IRS, other than institutions of higher education.
Additional Information on Eligibility
Foreign entities are not eligible to compete for, or receive, awards made under this announcement. Faith-based and community organizations that meet the eligibility requirements are eligible to receive awards under this funding opportunity announcement.
