The Community Foundation for Staffordshire invites community organisations from across Stoke-on-Trent to apply.
Communities Together has a vision for Stoke-on-Trent that all communities are vibrant and inclusive – where people are aspirational and feel a sense of belonging and pride in their places.
This programme is a locality-based community support programme in partnership with Adult Social Care, aimed at supporting people being discharged from hospital and reducing the likelihood of being readmitted to hospital.
An opportunity for hyperlocal community organisations to enhance the current locality offer and work alongside social care to support local people in their community.
Funding Information
£5000 – larger grants may be available on discussion with the Programmes Team. Groups will be given 12 months to deliver the project.
Beneficiaries
This is an Adult Social Care programme and so beneficiaries must be Adults aged 18 and over.
Projects could include, but not limited to:
- Food parcels/ food classes and budgeting
- Transport home from hospital/ settle in service at home after being discharged from hospital / drop in service once discharged from hospital.
- Furniture removal / house clean/ removal of hoarding
- Handy person service / equipment delivery (if no family /friend network available)
- Support with shopping
- Support to attend health appointments
- Support with medication collection and arranging future delivery with pharmacies
- Face to face befriending
- Community Lounges
A community lounge provides a place to gain local advice and support available, including: Budgeting skills, employment and training, volunteering, community activities, mental health and wellbeing, fuel poverty, housing, benefit support, carer support and independent living.

The Fund will support organisations based in, or working for the benefit of people, in the following local authority areas;
- Stoke-on-Trent
Eligibility Criteria
- Constituted voluntary groups
- Constituted Community groups
- Registered Charities
- Social Enterprises (with an asset lock)
- Groups must be established for more than 12 months.
- Groups must be locally based with strong local links. They must also have capacity to link with Locality Connectors and deliver the support required.
- Priority will be given to organisations currently running community lounges or those who would be interested in running a community lounge, especially in the following areas:
- Trent Vale
- Stoke
- Shelton
- Joiners Square
- Hartshill
- Fenton
- Meir
- Smallthorne
- Sneyd Green
- Tunstall
- Longton
- Normacot
- Trentham
