The Centers for Disease Control is soliciting non-research, cooperative agreement applications to improve health outcomes among children and adolescents affected with a congenital heart defect (CHD) and identify factors contributing to health disparities.

This NOFO supports population-based surveillance of children with CHD to examine childhood and adolescent outcomes as well as needs and experiences of their caregivers. High-quality data will improve understanding of the epidemiology and public health significance of CHD. Surveillance data will also allow monitoring of special sub-populations with CHD (e.g., racial/ethnic, sociodemographic diversity), and potentially identify factors to inform development of effective secondary prevention strategies.
