The Hertfordshire Community Foundation (HCF) is seeking applications to provide services and activities which help improve the lives of older people and adults with disabilities in Hertfordshire.

The Adult Care Services (ACS) Commissioning team wants to work closely with the voluntary sector to improve the quality of life of older people and adults with disabilities in Hertfordshire. These grants are designed to fund community based activities which focus on Hertfordshire’s Connected Lives approach. Connected lives are about helping people to live the life they want. It’s not just about connecting people to other people, but connecting them to communities, networks, employment and volunteering opportunities, technology and equipment. It is a strong tool in combating loneliness.

Services and activities funded by these grants will also play a pivotal role in preventing or reducing reliance on formal care services. ACS wants to ensure a joined-up support and care network for those who need health and adult social care including older people and working age adults with disabilities.
