The Kent County Medical Society Alliance (KCMSA) has launched its Mini Grant Program to support and promote the health and well-being of individuals, programs and organizations in greater Grand Rapids.
Funding Information
Each Mini Grant request should not exceed $3,000.
Eligibility Criteria
Applications are received on an on-going basis from organizations with a 501(c)(3) status. Individual requests may be made through an organization that meets this requirement.
- Considerations are given to requests that meet the following criteria:
- Provide health education, medical services, medical supplies;
- Promote social welfare;
- Demonstrate a relationship between the proposal and the goal of the KCMSA Foundation;
- Are located in greater Grand Rapids;
- Are $3,000 or less;
- Are a 501(c)(3) nonprofit organization;
- Meet reporting requirements as stated on the application;
- Spend the grant dollars within one calendar year of receipt.
Application Requirement
Type and attach in the following order. Include any additional information that would aid their decision:
- Create a cover letter by copying this page and filling out the information below.
- Describe the project/proposal. Include the purpose, need, target population and geographic area your organization will address with funds from this grant.
- List the names and qualifications of the individuals who will implement this project/proposal.
- Attach project budget. Be specific.
- How does your project/proposal meet the goals of the Foundation?
- Give a one page or less description of your organization.
- Attach your organization’s budget for the current year.
- Attach your organization’s 501(c)(3) IRS tax identification letter.
