Applications are now open for the CareOregon Emergency Relief Fund to provide one-time, immediate support to CareOregon network partners experiencing financial hardship due to an emergency event outside of their control.
Eligibility Criteria
In order to be eligible for the Emergency Relief Funds, provider agrees to:
- Respond to all application questions
- Provide brief accounting of other funds secured (federal, state, local, foundation, etc.)
- Submit report within 6 months of receipt of funds detailing how dollars were used and their impact
- Complete and sign attestation in Documents tab of this application
Restrictions
If applicant is not a tax exempt organization an additional signed LOA may be necessary. More information will be provided to these applicants.
