Applications are now open to Address Social Determinants of Health.
Factors that influence health are known as social determinants of health (or social influencers to health). They are the conditions and environments, in which people are born, live, learn, work, play, worship and age.
Quality of health can be traced to social determinants of health. The priority is to help Idahoans live and enjoy healthier lives.
Funding Priorities
They are looking to assess the impact of integrating, embedding, and partnering healthcare and community-based organizations to address social determinants of health (SDOH) through:
- Demonstration Projects: Assist in launching and assessing new innovative solutions to address SDOH.
- Evaluation of Existing Efforts: Determine the efficacy of current efforts addressing SDOH.
Funding Information
- The Blue Cross of Idaho Foundation for Health will be awarding grants up to $100,000 to address social determinants of health (SDOH).
Eligibility Criteria
- 501(c)3 organizations, including nonprofit health clinics and community organizations
- Tax-exempt universities and colleges
