The Department of Health and Human Services (DHHS) and Office of the Assistant Secretary for Health is seeking applications for the Accessing Social Determinants of Health Data through Local Data Intermediaries Initiative.
It is increasingly recognized that local environmental and various socioeconomic factors, also known as SDOH or social needs, are critical contributors to health and well-being at the neighborhood level.
SDOH are the conditions in the environments where people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning, and quality-oflife outcomes and risks.
Healthy People 2020 developed a “place-based” organizing framework reflecting five domains:
- Economic Stability
- Education
- Social and Community Context
- Health and Health Care
- Neighborhood and Built Environment
Research has shown that one way to overcome the lack of geographically granular data at the local level is to link datasets, including health and SDOH/social need/social risk data, from various local sources.
However, there are challenges for supporting the integration of and access to shared SDOH/social need/social risk and health data, including lack of:
- robust data sharing infrastructure;
- data standards for SDOH; and
- interoperability among data systems.

- Estimated Federal Funds Available: $ 500,000
- Award Ceiling (Federal Funds including indirect costs): $250,000 per budget period
- Anticipated Number of Awards: 2
- Anticipated Start Date: September 30, 2021
- Estimated Period of Performance: Not to exceed 3 years
- Anticipated Initial Budget Period Length: 12 months
Eligibility Criteria
- Any public or private nonprofit entity located in a State (which includes one of the 50 United States, District of Columbia, Commonwealth of Puerto Rico, U.S. Virgin Islands, Commonwealth of the Northern Mariana Islands, American Samoa, Guam, Republic of Palau, Federated States of Micronesia, and the Republic of the Marshall Islands) is eligible to apply for an award under this announcement.
- Faith-based organizations and American Indian/Alaska Native/Native American (AI/AN/NA) organizations are eligible to apply. Examples include:
- State Governments
- County Governments
- City or township governments
- Special district governments
- Independent school districts
- Public and State controlled institutions of higher education
- Native American tribal governments (Federally recognized)
- Public Housing authorities/Indian housing authorities
- Native American tribal organizations (other than federally recognized tribal governments)
- Nonprofits having 501(c)(3) status with the IRS, other than institutions of higher education
- Nonprofits without 501(c)(3) status with the IRS, other than institutions of higher education
- Private nonprofit institutions of higher education
- U.S. territories
