The Health Resources and Services Administration is seeking applications for its National Center for a System of Services for Children and Youth with Special Health Care Needs Program.
Program Goals
- The goal of the National Center is to advance and strengthen the system of services for CYSHCN, their families and caregivers at the community, state, and national levels. This will be achieved by leading the field and advancing the system of services so that care and other supports are integrated, family-centered, evidence-informed, and culturally responsive.
Program Objectives

To monitor and evaluate the effectiveness of the program, the recipient will be responsible for collecting data and annually reporting on these objectives:
- By June 30, 2025, the Blueprint Implementation Roadmap will be developed and disseminated to all sectors3 serving CYSHCN and their families.
- By June 30, 2028, pilot at least 5 innovations to implement the Blueprint for Change.
- By June 30, 2028, at least 25 entities4 engaged with the National Center through TA and training will have implemented a principle or strategy of the Blueprint Implementation Roadmap.
- By June 30, 2028, at least 25 states/jurisdictions will document utilization of Blueprint for Change principles/strategies in their Title V Annual Report, as evidenced by activities or the development of state performance measures or other type of metric.
