The Rocky Mountain Health Foundation expects to distribute $1 million in 2018 throughout quarterly grants. Grants will be awarded to organizations and programs that address access to health care, behavioral health services, community based projects and programs that integrate health systems and social services programs. Grants are awarded quarterly. 2018 deadlines are January 24, April 18, July 18, and October 24.
Rocky Mountain Health Foundation will consider one year funding commitments for general operating and programmatic requests from qualified 501(c)(3) nonprofit organizations who serve the 22 counties defined in the traditional maps of Colorado’s Western Slope (Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, La Plata, Lake, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit)
Grants will be awarded to organizations and programs that address: Access to healthcare-including telehealth, Preventive care and services, Behavioral health services, Case management, Population health (including integrating social service programs and health systems), Community-based projects.
The grant application provides the opportunity for applicants to introduce their organization, tell the foundation about the community they serve, what community health assessment gap the applicants are addressing, and how foundation can help. It will also allow foundation to determine whether the purpose or project for which applicants are seeking funding, aligns with foundation’s mission and values.
Rocky Mountain Health Foundation improves the health of western slope Coloradans serving as a catalyst and collaborator for innovative healthcare approaches, and promoting the health and well-being of Western Slope communities. “Well-being” includes not only physical and behavioral health-related issues, but the factors that can affect one’s health—or the social determinants of health.
