The Weld Trust (TWT) has launched its Health Grants Program to promote health and education for the residents of Weld County.
Focus Area
- In accordance with The Weld Trust’s vision to help Weld County become the healthiest County in the State of Colorado, the fourth quarter 2021 Health initiative is focused on:
- Food Security
- The Weld Trust will support efforts by organizations or collaborations that address the needs of the most vulnerable population experiencing food insecurity.
- The Weld Trust will fund collaborative programs and resources that put nutritious food into the hands of people in need across Weld County.

Project Priorities
In addition, The Weld Trust gives priority to projects that:
- Address underlying social and economic challenges contributing to poor health outcomes in the communities served, such as poverty.
- Extend past individual behavior or program-related services by modifying policies, systems, and environments to help make healthy options practical and readily available.
- Demonstrate collaboration among sectors, including public health, government, healthcare, etc. and among multiple nonprofit agencies/institutions/departments/governmental entities.
- Prioritize engagement of residents from the communities served.
- The Weld Trust values evidence-based best practices, but also encourages innovative solutions.
- Leverage additional funding as well as community assets.
- Have a demonstrable sustainability plan that will enable the work to continue beyond the funding period.
- Provide an established plan for evaluation including measurable outcomes. Explain the way in which you will know that you have been successful in achieving the impact you intended. Identify the specific manner in which the outcomes you propose impact the funding strategy of TWT.
Eligibility Criteria
Eligible applicants must be tax-exempt organizations under Internal Revenue Service 501(c)(3) or operate under the fiscal sponsorship of a tax-exempt nonprofit, including schools and public organizations.
Note: Only nonprofit organizations, schools and government agencies located in Weld County or whose program is exclusively for Weld County residents in the areas of health or education are eligible to apply.
