The Healthcare Georgia Foundation is currently seeking proposals for its The Two Georgias Initiative to support the statewide establishment of rural Community Health Partnerships (CHP) with the vision and capacity to implement Community Health Improvement Plans (CHIP) that achieve measureable and sustainable improvements in rural health equity.
The Two Georgias Initiative (Initiative) is a Healthcare Georgia Foundation place-based grantmaking program designed to achieve greater health equity among rural Georgians through the elimination of health disparities. The Initiative is a multi-year investment in rural communities represented by Community Health Partnerships (CHP) implementing bold and visionary Community Health Improvement Plans (CHIP) comprised of innovative solutions to better health and healthcare among their residents. Strengthening community, organizational, and individual leadership, the Partnerships will pursue small steps toward health equity with a long-term cadence.
The Initiative recognizes the need for short-term results, yet requires the stamina for long-term impact. The Initiative positions rural communities to embrace local autonomy (flexibility and choice), while seeking strategic opportunities to coordinate and align with others. The Community Health Improvement Plan will demand rigor, insist on evidence, and despite risks create opportunity for innovation. The Partnership’s plan will maintain some top-down strategy and direction and also be willing to harness bottom-up opportunities. The Plan will embrace collaboration in planning and in execution.
Program Goals
The goals of The Two Georgias Initiative are to:
- Achieve greater health equity among rural populations;
- Improve health and healthcare for rural Georgians;
- Build healthier rural communities;
- Improve social conditions that impact health of rural population;
- Build community, organizational, and individual leadership capacity in rural Georgia
Funding & Duration
Applicants may apply for up to $70,000 for a grant period of 12 months.
Eligibility Criteria
- The Foundation will accept proposals from Georgia-based non-profit organizations, nonprofit health organizations, hospitals, and governmental agencies with a 501(c) (3) designated lead organization representing a rural community health partnership.
- Organizations that are not a 501(c) (3) entity should apply through a tax-exempt organization acting as a fiscal sponsor.
- If an organization is not a 501(c) (3) entity and plans to use a fiscal sponsor, that sponsor must send a cover letter with the application indicating it has agreed to support the community health partnership and it’s health improvement plan.
- Pre-approval of the fiscal sponsor is necessary for the Foundation to proceed with the funding request.
- The Foundation does not make grants for activities that exclusively benefit the members of sectarian or religious organizations.
- The Foundation will not accept more than one application per eligible organization or rural community health partnership.
